CONCLUSIONES• En los últimos años ha habido un aumento en el uso de VMNI y una reducción en el uso de
corticoides postnatales
• Estos cambios no se han acompañado de una clara y consistente mejoría en el pronóstico
respiratorio ni a corto ni a largo plazo
• Tampoco se ha observado una mejoría clara en el pronóstico neurológico
• Existen dudas razonables para trasladar directamente los resultados de una población al resto
• Los datos presentados merecen un análisis profundo y constituyen una alerta para la evaluación
de las estrategias ventilatorias actuales en recién nacidos extremadamente prematuros

• La cafeína en prematuros es un tratamiento seguro y eficaz
• Mejora el pronóstico respiratorio a corto y a largo plazo
• Mejora el pronóstico neurológico
• No están justificados más ensayos de cafeína vs placebo
