Fisiopatología Metabolismo de lípidos y glucosa
Lipotoxicidad y resistencia a la insulina en obesidad
• El almacenamiento de grasa corporal es requerida
• Exceso del mismo resulta en obesidad
• Exceso Ácidos grasos libres – estrés oxidativo
![Page 9: 21.obesidad](https://reader036.fdocumento.com/reader036/viewer/2022062303/558c2cb4d8b42ab2738b46b2/html5/thumbnails/9.jpg)
Gastroenterol Hepatol (N Y). 2007 November; 3(11): 856–863.
• Triacilglicerol se almacena en exceso
• Inhibe lipogenesis previniendo adecuada depuración produciendo hipertrigliceridemia
• Liberación de ácidos grasos por la lipasa lipoproteína aumenta los niveles de trigliceridos
• Niveles elevados de β lipoproteinas causa lipotoxicidad que resulta en resistencia a insulina
![Page 10: 21.obesidad](https://reader036.fdocumento.com/reader036/viewer/2022062303/558c2cb4d8b42ab2738b46b2/html5/thumbnails/10.jpg)
Gastroenterol Hepatol (N Y). 2007 November; 3(11): 856–863.
• Hiperglucemia compensada con gluconeogenesis
• Disminuyen la utilización de la insulina estimulada por el musculo
• Lipotoxicidad disminuye producción insulina células β
![Page 11: 21.obesidad](https://reader036.fdocumento.com/reader036/viewer/2022062303/558c2cb4d8b42ab2738b46b2/html5/thumbnails/11.jpg)
Gastroenterol Hepatol (N Y). 2007 November; 3(11): 856–863.
Adipocitos
• 1 billon de células
• Prohormonas:– Lectina– Adiponectina– Visfatina
• Regular la grasa corporal
![Page 12: 21.obesidad](https://reader036.fdocumento.com/reader036/viewer/2022062303/558c2cb4d8b42ab2738b46b2/html5/thumbnails/12.jpg)
Gastroenterol Hepatol (N Y). 2007 November; 3(11): 856–863.
• Grasa visceral libera adipocitos inflamatorios
• Transportados por el sistema porta causando esteatohepatitis no alcohólica
• Lesión inflamatoria que con lleva disfunción de células β
![Page 13: 21.obesidad](https://reader036.fdocumento.com/reader036/viewer/2022062303/558c2cb4d8b42ab2738b46b2/html5/thumbnails/13.jpg)
Gastroenterol Hepatol (N Y). 2007 November; 3(11): 856–863.
• Adipocitos liberan renina, angiotensinogeno y antiotensina II
• TNF-α aumenta en proporción a la grasa corporal total causando hígado graso
• El transportados GLUT 4 es afectado
![Page 14: 21.obesidad](https://reader036.fdocumento.com/reader036/viewer/2022062303/558c2cb4d8b42ab2738b46b2/html5/thumbnails/14.jpg)

Anti-inflamatorios
• Aiponectina/visfatina
– Anti inflamatorios– Anti aterogenicos– Aumentan sensibilidad de insulina– Mejoran la disfunción endoteliar vascular– Mayor obesidad menor adiponectina
