0,032), mayor peso al nacer neonatal (p = 0,007)

Conclusiones
• La principal limitación para el uso de la glibenclamida fue la falla en lograr un optimo control glicémico (4% a 47%)– La probabilidad de falla es mas alta con el incremento de la edad
materna, alta paridad, diagnostico de DG antes de las 30 ss, glucosa en ayunas mayor 100 mg/dl y un TTOG mayor de 200.
• Los efectos de glibenclamida en la seguridad fetal y sus consecuencias a largo plazo todavía no se conocen.
• Hay una clara necesidad de realizar un ensayo clínico controlado suficientemente grande con la potencia adecuada para evaluar los riesgos y beneficios del uso de glibenclamida en GDM.

Se sugiere un mejor resultado materno (menos aumento de peso, menos
hipertensión inducida por el embarazo y preeclampsia) en el grupo tratados con
metformina en comparación con insulina.
En los resultados neonatales se observo menor hipoglucemia neonatal en el brazo
de metformina.
También se observo nacimientos prematuros significativamente mayores en
las madres que toman metformina en comparación con la insulina

Metformina en gestantes con SOP• La metformina es conocida por facilitar la concepción
en mujeres con SOP.– Vanky et al (2004) encontró reducción significativa de las
complicaciones relativas al embarazo comparado con placebo. (0 vs 32%; p = 0.01)
– Begun et al (2009) encontro 9 veces menos riesgo de desarrollar Diabetes gestacional.
– Vanky et al (2010) encontro menos partos preterminos usandolo desde el primer trimestre (3 vs 11% p < 0.01)

CONCLUSIONES
• El mayor limitante para el uso de metformina es dificultad para llegar a un buen control glicémico ( 10% - 46%) y que las pacientes requieran insulina adicionalmente.
• El requerimiento extra de insulina fue 11,5 veces mas probable con el uso de metformina en lugar de glibenclamida.– Los factores de riesgo para la suplementación con insulina serian: la
raza, el IMC y la glucosa basal.• El impacto del uso de metformina a largo plazo en el neonato
es incierto.
