Figure 2 shows an ROC space of the individual studies, aswell as the bootstrapped sROC curve plot for all studiesin asymptomatic women with a twin or triplet pregnancy.

Studies in which cervical length was measured before20 weeks show an overall high specificity, but sensitivityin these studies does not exceed 70%. The overall sROCcurve shows a moderate accuracy, with sensitivity beingslightly better than specificity.
Figure 3 shows summary point estimates for sensitivityand specificity with 95% CIs for four different cut-offs

tween 0.56-1.17, respectively, to predictspontaneous preterm birth from !32 to!37 weeks of gestation in asymptomaticwomen by using cutoff values for CLranging between 20-35 mm at 20-24weeks of gestation. Pooled positive andnegative likelihood ratios for predictingpreterm birth at !34 weeks of gestationin asymptomatic women using cutoffvalues for CL ranging between 25-35mm at "24 weeks of gestation varied be-tween 1.8-2.1 and between 0.29-0.83, re-spectively. This review, however, in-cluded 2 studies with duplicate data and1 study in which a cervical cerclage wasplaced in women with a CL !30 mm at!27 weeks of gestation.
The results of our systematic reviewsuggest that transvaginal sonographicmeasurement of CL is a better predictorof spontaneous preterm birth in twinpregnancies than in singleton pregnan-cies. In fact, the metaanalysis by Honestet al4 reported that among asymptomaticwomen with singleton pregnancies, a CL!25 mm at 20-24 weeks of gestation hadpooled positive and negative likelihoodratios of 4.2 and 0.40, and 4.4 and 0.67,respectively, to predict preterm birth!32 and !34 weeks of gestation, respec-
tively. At !20 weeks of gestation, a CL!25 mm had pooled positive and nega-tive likelihood ratios of 4.1 and 0.75, and6.3 and 0.79, respectively, to predict pre-term birth !32 and !34 weeks of gesta-tion, respectively, whereas at "24 weeksof gestation the pooled positive and neg-ative likelihood ratios for predicting pre-term birth at !34 weeks of gestationwere 4.1 and 0.62, respectively. In con-trast, the present study showed thatamong asymptomatic women with twinpregnancies, a CL !20 mm at 20-24weeks of gestation had pooled positiveand negative likelihood ratios of 10.1 and0.64, and 9.0 and 0.64, respectively, topredict preterm birth at !32 and !34weeks of gestation, respectively.
Although our metaanalyses have dem-onstrated that transvaginal sonographicmeasurement of CL is predictive of pre-term birth in asymptomatic women withtwin pregnancies, it is unclear if antena-tal management of these pregnanciesbased on the results of this test can pre-vent preterm birth. In 2006, Gordon etal42 conducted a study in which 125women with twin pregnancies were ran-domly assigned to undergo a transvagi-nal sonographic CL measurement and a
cervical digital examination every 4weeks starting at 16-20 weeks until 28weeks of gestation (n # 63) or to a digitalcervical examination without cervical as-sessment by ultrasound at the same in-tervals (n # 62). Women who under-went transvaginal sonographic cervicalexaminations were treated with a prede-termined algorithm for the use of cer-clage and bed rest. Treatment decisionsof women allocated to the control groupwere not based upon a predetermined al-gorithm. There was no significant differ-ence between groups in mean length ofgestation (35.7 $ 2.2 weeks in the studygroup vs 35.5 $ 3.1 weeks in the controlgroup, P # .60) but life table analysis re-vealed that preterm birth !35 weeks ofgestation in the transvaginal sono-graphic CL group was significantly re-duced (P # .02). In addition, it should beemphasized that as of late 2009, there is notherapy that effectively prevents pretermbirth in twin pregnancies. This includesthe use of bed rest,43 oral betamimetics,44
cervical cerclage,45 and progesterone.46
Further well-designed randomizedcontrolled trials are required to evaluatethe effectiveness of antenatal treatmentof women with twin pregnancies based

patients in group A delivered at significantly earlier gestationalages compared with the patients in group B (Table 2). Thedifference between the groups remained statistically significanteven after excluding data from patients who presented with a shortcervix (CL ! 25 mm) on the second ultrasound assessment of theCL (Tables 2 and 3).
Multiple logistic regression model was performed, controllingfor age, parity, gestational diabetes, confirmed urinary tractinfection, prior PTB, in vitro fertilization and chorionicity. Cervicalshortening of "25% was the only factor significantly associatedwith spontaneous PTB with a gestation time of <32 weeks (crudeOR (95% CI) 14.71 (6.25–36.18); P-value < 0.001, adjusted OR (95%CI) 20.26 (7.93–56.91); P-value < 0.001). This result is still trueafter excluding the women with a CL ! 25 mm on the secondultrasound assessment of the CL.
There was a >7-fold increase in the rate of spontaneous pretermbirth at <32 weeks, from 4 to 28.6% (OR, 9.54; 95% CI, 3.32–27.36)(Table 2). This result is still true after excluding the women with aCL ! 25 mm from 3.5 to 26.3% (OR, 9.76; CI, 2.64–36.04) (Table 3).
The use of CL shortening by "25% has better sensitivity than theuse of CL ! 25 mm for the prediction of PTB at <28, <30, <32, and<34 weeks of gestation, respectively, but the prediction specificityis higher for CL!25 mm for the same gestation periods (Table 4).The sensitivity, specificity, and positive and negative predictivevalue of CL shortening by "25%, were 58.8%, 87%, 28.57%, and95.97%, respectively, for the prediction of spontaneous PTB at <32weeks gestation. Based on ROC curve analysis, the use of CL shortening by "25%was superior, but not statistically significantly, to the use of CL forthe prediction of PTB at <32 (Fig. 1 graph C, P = 0.0524), and at <34weeks (Fig. 1 graph D, P = 0.281). CL, however, was preferred for theprediction of PTB at <28 (Fig. 1 graph A, P = 0.037) and <30 weeks(Fig. 1 graph B, P = 0.0457).
4. Comment
This study confirmed the association between the shortening ofCL by "25% over two measurements and PTB in asymptomatic twinpregnancies. This test was found to be a good predictor of PTB at <28,<30, <32, and <34 weeks’ gestation. This result remained true evenafter excluding women with a short cervix (CL ! 25 mm) on the
repeated CL. Our results are similar to and consistent with those ofFox et al. [7], who found that a CL that decreases by "20% over 2measurements is a significant predictor of spontaneous pretermbirth at <28, <30, <32, and <34 weeks. This effect remained trueeven when excluding patients who exhibited a CL ! 25 mm.
With the use of CL shortening by "20% in the Fox et al. study [7],the sensitivity was 75, 60, 54.5, and 35%, and the specificity was86.3, 86.2, 88.1, and 88.1% for the prediction of PTB < 28, <30, <32,and <34 weeks respectively. Our cutoff point of CL shortening by"25% is better for the prediction of PTB < 32 and <34 weeks thanthe "20% cutoff point used by Fox et al. [7], but the "20% cutoffpoint is better for the prediction of PTB < 28 and <30 weeks.
Our study differs from both Fox et al. [7], and other previousstudies [2–6] in that we evaluated both the CL and the percentchange in CL over time to predict PTB. There are several studies thatexamined the use of single or repeated cervical length measurementas a predictor of PTB in twin gestations. Although predictiveaccuracy varies between studies, shortened cervical length usuallyhas a low sensitivity but high positive predictive value for PTB [2–6].
Goldenberg et al. [3] studied 147 sets of twins and found thatthe rates of delivery at <32 weeks of gestation were 26.9% and13.2% when the cervix measured !2.5 cm at 24 and 28 weeks ofgestation, respectively [3]. We almost have similar results, but thenumber of patients with short CL ! 25 mm in our study were only20 cases in total, which is smaller than the number in theGoldenberg et al. [3] study with 26 cases at 24 weeks and 48 casesat 28 weeks.
Souka et al. [4] found that a cervical length of !25 mm at 23weeks had sensitivities for predicting spontaneous PTB at <28, <30,<32, and <34 weeks’ gestation of 100, 80, 47, and 35%, respectively.Although our established variable of CL shortening by "25%between the two measurements have better sensitivity than theuse of the CL by Souka et al. [4] to predict PTB at <32 and 34 weeks, itwas not as effective as Souka et al.’s method at predictingspontaneous PTB at <28 and <30 weeks’ gestation. It is possiblethat the small number of patients who were delivered at <28 weeks(4 patients) and <30 weeks (5) acted as a limiting factor in our study.
In our study the high specificities and the high negativepredictive values of CL shortening by "25% for the prediction ofspontaneous PTB at <32 weeks’ gestation indicate that CLshortening was better at predicting the absence than the presenceof various degrees of spontaneous prematurity. Therefore, patientscan be informed and reassured that the risk of very PTB is quite low,potentially avoiding hospitalization, bed rest, and overtreatment.

Cervicalshortening of "25% was the only factor significantly associatedwith spontaneous PTB with a gestation time of <32 weeks (crudeOR (95% CI) 14.71 (6.25–36.18); P-value < 0.001, adjusted OR (95%CI) 20.26 (7.93–56.91); P-value < 0.001). This result is still trueafter excluding the women with a CL ! 25 mm on the secondultrasound assessment of the CL.
There was a >7-fold increase in the rate of spontaneous pretermbirth at <32 weeks, from 4 to 28.6% (OR, 9.54; 95% CI, 3.32–27.36)(Table 2). This result is still true after excluding the women with aCL ! 25 mm from 3.5 to 26.3% (OR, 9.76; CI, 2.64–36.04) (Table 3).

Subgroup and sensitivity analysesSubgroup analyses of the effect of vaginalprogesterone on primary outcomes arepresented in Table 5. There was no evi-dence that women in any one of the pre-specified subgroups benefit more or lessfrom the use of vaginal progesteronethan those in any other subgroup (all Pfor interaction " .30). However, the useof vaginal progesterone was associatedwith a statistically significant reductionin the risk of preterm birth !33 weeksand composite neonatal morbidity and
mortality in both women with no previ-ous spontaneous preterm birth andwomen with at least 1 previous sponta-neous preterm birth !37 weeks of gesta-tion, women with a sonographic cervicallength between 10-20 mm, women aged20-34 years, and Caucasian women.
No significant differences were notedfor preterm birth !33 weeks of gestationand composite neonatal morbidity andmortality between subgroups based onthe daily dose of progesterone. A signifi-cant decrease in the risk of preterm birth!33 weeks of gestation and compositeneonatal morbidity and mortality wasfound in women who received either 90-100 or 200 mg/d of vaginal progesterone.
The effect of vaginal progesterone onthe risk of preterm birth !33 weeks of
gestation and composite neonatal mor-bidity/mortality did not change whensensitivity analysis was limited to the 2trials131,133 in which the primary aim wasto evaluate the effect of vaginal proges-terone in women with a short cervix(pooled RR, 0.57; 95% CI, 0.40 – 0.80 forpreterm birth !33 weeks and pooled RR,0.54; 95% CI, 0.35– 0.82 for compositeneonatal morbidity/mortality). In addi-tion, the results of the metaanalyses didnot change significantly when randomeffects models were used for pretermbirth !33 weeks (RR, 0.59; 95% CI,0.43– 0.81) or for composite neonatalmorbidity/mortality (RR, 0.59; 95% CI,0.41– 0.83). Sensitivity analyses based ontrial quality were not performed becauseall trials were considered at low risk for

+Conclusiones
n Existe poca evidencia para uso cualquier método en prevención PP Embarazo Gemelar.
n Progesterona vaginal en embarazo gemelar con LC< 25 mm asintomático, podría mejorar outcome neonatal sin disminuir incidencia PP. Faltan estudios con mayor N.
n Cerclage en embarazo gemelar con LC< 25 mm asintomático podría ser útil en prolongar el periodo de latencia al parto.
n Cerclage de emergencia en embarazo gemelar permitiría prolongar la gestación hasta la viabilidad.
n Pesario resultados controvertidos: n Podría disminuir riesgo PP < 32 sem en casi 50% en mujeres con LC < p
