CATEGORIAS CLINICAS.
1- Efecto del embarazo normal, sobre la función hepática materna.
2- Enfermedades hepáticas relacionadas específica o particularmente con el embarazo.
3- Efectos del embarazo sobre las enfermedades hepáticas agudas o crónicas no relacionadas y viceversa.

1-EMBARAZO SOBRE EL HIGADO.
• Alteraciones anatómicas. ( Variación del tamaño y forma de los hepatocitos e infiltrado linfocítico portal).
• Alteraciones fisiológicas. del vol/min circulatorio.
• Alteraciones de la función hepática. (Disminución de albúmina y Gamma,
aumentan factores VII, VIII, IX y X, ceruloplasmina, F Alcalina, Lípidos.)

EFECTO DE LAS HORMONAS.
• Pueden causar colestasis ( Estasis biliar centrolobulillar, asociada a dilatación de los canalículos y pérdida de la estructura microvellosa normal de los mismos.
• Fosfatasa Alcalina, 5´Nucleotidasa, GGT, Acidos Biliares y a veces Bilirrubina

2-ENF HEPATICAS RELACIONADAS CON EL EMBARAZO.
1- Colestasis intrahepática del embarazo.
2- Enfermedad hepática pre-eclámptica.
3- Síndrome HELLP.4- Hematoma y ruptura hepática.5- Infarto Hepático.6 Hígado Graso Agudo del
Embarazo.

C.CLINICO COLESTASIS INTRAHEPATICA DEL EMBARAZO.
• Aparece en el tercer trimestre.• Prurito.• Coluria sin íctero franco.• Ictericia franca.• A veces hepatomegalia leve y sensible.• Aparición precoz se asocia a sufrimiento
fetal.• Cede con el parto.• Recurre en otros embarazos.• Posible causa genética.

TRATAMIENTO COLESTASIS INTRAHEPATICA
DEL EMBARAZO.
• Tratamiento del prurito (Fenobarbital, Colestiramina, , S-adenosil-L-metionina, Acido Ursodesoxicólico).
• Inducción del parto ante evidencia o sospecha de Sufrimiento Fetal.

ENFERMEDAD HEPATICA PRE-ECLAMPTICA.
• SINDROME HELLP.• HEMATOMA HEPATICO CON
RUPTURA.• INFARTO HEPATICO. El implante de trofoblasto es anormal y lleva
a restricción del flujo sanguíneo a la unidad feto-placentaria que causa aumento de la resistencia vascular e hipoperfusión sistémica.

CUADRO CLINICO HELLP.
• Hemólisis leve con eritrocitos extraños.• Elevación de aminotransferasas• Plaquetopenia < de 100 000/mm3• Se encuentra en el 20% de las
preeclámpticas.• Dolor en hipocondrio derecho y tórax,
preeclampsia, náuseas y vómitos, segunda mitad del embarazo. Puede llegar a hematoma y ruptura hepática.

TRATAMIENTO DEL HELLP.
• Interrupción rápida del embarazo tanto por causa materna como fetal.
• Reposo y tratamiento anti-hipertensivo.

DIAGNOSTICO HEMATOMA Y RUPTURA HEPATICOS.
• Episodio catastrófico que provoca muerte materna y fetal elevadas.
• Shock.
• Dolor en hipocondrio derecho intenso.
• Hemorragia subcapsular.
• Hemorragia intraabdominal severa.

DIAGNOSTICO INFARTO HEPATICO.
• Pre-eclampsia y HELLP previos.• Dolor abdominal.• Fiebre.• Plaquetopenia• Elevación de ALAT > 5 000 u/l.• TAC- Areas sacabocados mal
vascularizadas.• Histol- Hemorragia con infiltración
neutrofílica peri infarto

TTO HEMATOMA, RUPTURA HEPATICA Y DEL
INFARTO..• Si sólo hematoma sin ruptura, Interrupción
del Parto urgente y/o Ingreso en UCI.• Si ruptura Cirugía Urgente. Sutura, parche de
epiplón, empaquetamiento, Gelfoam, lobectomía, ligadura de la arteria hepática, Transplante Hepático.
• Si Infarto: Interrupción del embarazo y medidas de sostén..

DIAGNÓSTICO HIGADO GRASO AGUDO DEL EMBARAZO (Sheehan).
• Comienzo segunda mitad del embarazo y postparto. Error Metabolismo(3HaCoADH)
• Náuseas, vómitos.• Malestar general, fatiga.• Ictericia. Dolor Abdominal.• Pre-eclampsia.• Coma e insuficiencia hepática.• Sangrado gastrointestinal y uterino. Muerte• Aumenta Bilirrubina, ALAT, GGT, Amoniaco,
Leucocitos y dismin fibrinógeno y glucosa.• Histol- Infilt grasa microvesicular en zona 3

TRATAMIENTO DEL HIGADO GRASO AGUDO
DEL EMBARAZO..
• 1- Parto. (Cesárea o Transvaginal).
• 2- Ingreso en UCI. Sostén.
• 3- Si fuera necesario, Transplante Hepático.
• Si se logra salvar, no deben quedar secuelas.

• 1- Hepatitis virales A, B, C, D, E, GBC, SEN V, TTV, HERPES TIPO II.
• 2- Hepatitis tóxicas ( Medicamentos y alcohol).
• 3- Cirrosis Hepática.
• 4- Trasplante Hepático.
• 5- Enfermedad de Wilson.
• 6- Síndrome de Budd-Chiari.

• Típica o ictérica.
• Anictérica.
• Inaparente o asintomática.
• Recurrente o polifásica.
• Colestásica.
• Sobreaguda o subfulminante.

• Fecal-oral. Hídrica.
• Epidemias-pandemias.
• Período de incubación 15-50 días

• IgM anti HAV marcador viral.

• Transmisión sexual, parenteral y perinatal.
• Periodo de incubación 15-160 días.
• Mortalidad 0,2-1%
