Enfermedad de la colágena.
>mujeres, edad fértil, > raza negra. Edad:
Pre-puberes (2:1) Adolescentes (5:1) Adultos (12:1)
Agudo – lento.
Multifactorial: Factores ambientales. Estrógenos. Desregulación inmune. Hormonal. Microorganismos, Luz ultravioleta (actividad)
Genética.
LES
Los genes de ciertos antígenos leucocitarios, componentes del complemento, receptores de inmunoglobulinas entre otras proteínas han sido asociados.
Estado de inmunidad generalizado. Autoanticuerpos:

Nefritis Lúpica
Es una de las manifestaciones más serias de LES, usualmente 5 años después del dx.
Anti nucleosoma o anti DNA de doble cadena
Autoanticuerpos Cationicos (Carga) +patógenos.
Algunos isotipos que activan complemento (IgG1 e IgG3)
Patogenia
Agregados inmunes y complemento. Sitios de lesión glomérulo y en túbulos 2/3 de la
Bx.
No existe mucha diferencia entre los mecanismos patogénicos de daño renal de NL con GNP. Infiltrado mononuclear intersticial

precipitan desde la circulación y se acumulan en la membrana basal glomerular (GBM) en el espacio subendotelial o se forman ¡n situ en él.
B. Tinción inmunofluorescente de glomérulos con anti-lgG marcada, en donde se advierte la captación lineal en un individuo con enfermedad contra GBM, o los depósitos inmunitarios de un individuo con glomerulonefritis membranosa.
C. Los mecanismos de la lesión glomerular tienen una patogenia compleja. En forma clásica, los depósitos inmunitarios y los de complemento "atraen al glomérulo, macrófagos y neutrófilos. También atraen a los linfocitos T que participan en las características de la lesión.
D. Los mediadores de amplificación como los oxidantes provenientes del punto local o las proteasas amplifican dicha inflamación y según el sitio del antígeno "contra el que se actúa"y los polimorfismos genéticos del hospedador, se lesiona la membrana basal con proliferación endocapilar o extracapilar.

Manifestaciones renales del
LES
Manifestación universal: Proteinuria. Sx. Nefrótico (60%).
Hematuria microscópica.
Sedimento urinario (50%): Cilindros hialinos. Cilindros granulosos
eritrocitarios.
HTA.
IFG
25% NL al dx.
60% adultos y 80% niños la desarrollan al transcurso.

Histopatología
I Mesangial mínima
Arquitectura histológica normal con depósitosmesangiales.
II Proliferación Mesangial
Hipercelularidadmesangial con expansión de la matriz mesangial.
III Nefritis Focal
Proliferaciónendocapilar y extracapilar focales, con depósitos inmunitarios subendoteliales focales y expansión mesangial mínima.
IV Nefritis Difusa
Proliferación endocapilar y extracapilar difusa, con depósitos inmunitarios subendoteliales difusos y alteraciones mesangiales.
V Nefritis Membranosa
Engrosamiento de la membrana basal con depósitos inmunitarios subendoteliales difusos en el caso de lesiones de clase III o IV y a veces llamada nefritis membranosa y proliferativa mixta.
VI Nefritis esclerótica
Esclerosis global prácticamente en todos los capilares glomerulares.

urinary sediment; mild to moderate proteinuria (never nephrotic) but may have active serology
III. Focal proliferative LN (<50% of glomeruli involved)
More active sediment changes; often active serology; increased proteinuria (about 25% nephrotic); hypertension may be present; some evolve into a class IV pattern. Active lesion require treatment, chronic do not.
Active A/C: active and chronic ChronicIV. Diffuse proliferative LN (>50% of glomeruli involved); all may be with segmental or global involvement (S or G)

Tratamientos-NIH
Pacientes tratados con citotoxicos tienen menos falla renal a 10 años que los tratados con corticoesteroides.
Los esquemas de Ciclofosfamida IV mensuales a dosis altas (0.5 a 1g/m2) por 6 meses son mejores que la metilprednisolona para prevenir falla renal
Tratamientos-NIH
Ciclofosfamida mensual más metilprednisolona es más efectiva para prevenir falla renal que la monoterapia.
Micofenolato oral diario es mas efectivo que ciclofosfamida para inducir remisión de la nefritis lúpica severa.
