Parvovirus B19
• DNA virus, familia Parvoviridae.
• Afecta principalmente a escolares (eritema infeccioso o
quinta enfermedad), adultos generalmente asintomático.
• Contagio: secreciones respiratorias, contacto mano-boca, hematógena, transplacentaria.
• Seroprevalencia en embarazadas: 35 – 65%
• Inmunidad persistente NO reinfecciones
• Incidencia infección durante el embarazo 1 a 2%

• Riesgo de afectación fetal bajo (20% < 20 semanas), no se ha descrito

• Se replica en células progenitoras eritroides por lo que causa anemia

• 30% sin tratamiento
• 80 a 85% con tratamiento
• Descrito retraso neurodesarrollo y anomalías maduración cortical (secundario a hipoxia)
Muerte intrauterina
• 10 a 15% de los casos con infección < 20 semanas, en ausencia de hidrops.
• Mayor riesgo: infección entre 9 y 16 semanas.
• No se han descrito con infecciones maternas > 20 semanas

• Aparece 3 a 4 días post viremia (10-14 días post contagio).
• Persiste 3 a 4 meses.
IgG
• Se positiviza a los 7 a 14 días.
• Persiste toda la vida
Indicaciones serología
• Clínica materna compatible (SIEMPRE frente a exantema no vesicular)
• Marcadores ecográficos y PN aumentado persistente > 16 sem con cariograma normal
• Contacto de riesgo (cara a cara > 15 min)

Seguimiento ecográfico • Fundamental diagnóstico precoz de anemia fetal, ojalá pre
hidrops.
• Doppler VS-ACM sensible y específico para anemia fetal,
permite intervención precoz.
• Frecuencia del control
– Infección materna 9-24 sem: semanal desde las 16 y hasta 12 sem
de la infección materna, luego cada 2 semanas.
– Infección materna > 24 sem: cada 2 semanas por riesgo anemia
fetal mínimo + hidrops.
• Signos ecográficos menos específicos: hiperecogenicidad
intestinal, cardiomegalia, OHA/PHA, placentomegalia
(grosor > 4 cm).
• Fetos hidrópicos sobrevivientes neurosono y RM 32-34
sem (anomalías desarrollo cortical).

Terapia fetal
• VS-ACM > 1.5 MoM o signos de hidrops cordocentesis (si > 18 – 20
semanas) para evaluación anemia fetal y transfusión si Hto 30% o < -
2 DS.
– Transfusión GR en VU inserción placenta vs porción intrahepática.
– Objetivo: Hto fetal > 25%
– Segunda transfusion 10 días despues de la primera y evaluar la
necesidad de tranfusiones cada 3-4 semanas hasta 34 - 35 sem.
– 3.8 – 5.6% de mortalidad por procedimiento.
– Transfusión intraperitoneal: en casos seleccionados de hidrops
antes de las 18 – 20 semanas, sin comprobar grado de anemia
fetal.
• Sobrevida 90% aprox (depende si antes o después de
las 20 semanas, hidrops o no).

• Se ha observado mayor tasa de PP en VIH (+),
especialmente paciente con TARV y antecedente de TARV
previo.
• Si DU (+) incluso modificaciones cervicales “escasas”
AZT ev junto con tocolisis. Si fase activa o RPM cesárea.
• RPM:
– Riesgo TV 2% por cada hora de RPM (< 24 horas).
– Se desconoce riesgo de infección fetal si CV muy baja o TARV
conducta no clara.
– Según EG y Bishop, inducción inmediata si no hay contraindicación
para parto vaginal.

Conclusiones (VIH)
• Diagnóstico precoz, control prenatal
(ginecólogo + infectólogo) y conducta según
guías gran impacto TV.
• Enfasis en gineco-obstetra especialista dentro
del equipo.
• Importancia del parto mayor riesgo TV.
– Parto vaginal sólo si: embarazo bien controlado y
con TARV, CV < 1.000 copias/ml a las 34
semanas, parto de término, buenas condiciones
