Es una enfermedad infecciosa del
hígado causada por el virus y
caracterizada por necrosis
hepatocelular e inflamación. Puede
causar un proceso agudo o un proceso
crónico, que puede acabar en cirrosis
(pérdida de la "arquitectura" hepática
por cicatrización y surgimiento de
nódulos de regeneración) del hígado,
cáncer de hígado, insuficiencia
hepática e incluso la muerte.

La hepatitis B es causa importante de hepatitis crónica y carcinoma hepatocelular en
el mundo, con un periodo de incubación de 4-26 semanas, con una media de 6 a 8
semanas. Se puede contraer hepatitis B por medio de:
Relaciones sexuales con una persona infectada sin usar preservativo
Compartir agujas para inyectarse drogas
Hacerse un tatuaje o una perforación en alguna parte del cuerpo con instrumentos
sucios que se usaron con otras personas
Pincharse con una aguja contaminada con sangre infectada (el personal sanitario
puede contraer la hepatitis B de esta forma)
Compartir el cepillo de dientes o la máquina de afeitar con una persona infectada
Transfusiones sanguíneas

El virus de Hepatitis C (VHC) es el
principal agente causante de la
Hepatitis tipo no-A no-B a nivel
mundial. Actualmente existen 170
millones de personas portadoras del
virus en riesgo de desarrollar cirrosis
y cáncer hepático. La proteasa NS3
del VHC es esencial para la
replicación viral y es por tanto uno de
los principales objetivos para el
desarrollo de terapias específicas
contra la infección ya que además, es
codificada por uno de los genes con
menor variabilidad dentro del
genoma del VHC.

• El virus tiene frecuentes errores en la replicación → frecuente aparición
de mutantes
• La respuesta inmune resulta ineficaz frente a los mutantes
• El virus se asocia a la célula e inhibe la apoptosis
• Reacción inmunopatológica

- Difícil en formas agudas
- Serología en formas crónicas
- Detección de ARN viral
• Transmisión: Parenteral (menos
importantes las vías sexual, salvo si se padece otra ETS, y vertical, salvo en coinfectadas por VIH)
• Profilaxis:
- Control de transfusiones y transplantes
- No hay vacunas

¿Qué hacer para evitar el contagio ocupacional de hepatitis?
• Vacunación frente a VHB obligatoria
• Adopción rutinaria de precauciones universales:
-Lavado de manos (agua, jabón, desinfectante)
-Vestuario protector adecuado (guantes, mascarilla,…)
-Uso correcto de material desechable y reutilizable
• Medidas adicionales postexposición:
-Heridas o zonas de piel en contacto con sangre o fluidos corporales: Lavar con agua y jabón
-Contacto a traves de mucosas: Lavar con abundante agua o suero fisiológico

¿Qué hacer para evitar el contagio ocupacional de hepatitis?
• Medidas específicas:
1. Evaluar riesgo de Hepatitis B:
- Paciente como fuente de infección: Saber si es
HBsAg +/-. Si se desconoce se procederá como si
fuese +
- Persona accidentada: saber si está inmunizada o
no frente a VHB:
- Si tiene AntiHBs no hay que hacer nada.
- Si no tiene AntiHBs: Inmunoglobulina
específica VHB e iniciar vacunación
