La Escalera Analgésica de la OMS cumple 20 años.
Historia y desafíos a futuro.
Aspectos históricos.-
A comienzos de la década de los 80 la situación de los pacientes de
cáncer avanzado y dolor era muy triste y penosa. En los paises
desarrollados muchas voces se habían alzado denunciando esta situación,
pero en los paises del tercer mundo la realidad era aún peor. Millones de
pacientes con cáncer no tenían acceso a ningun tratamiento y menos aún
al alivio del dolor y los síntomas agregados (2).
En esa época el Dr. Jan Stenward, oncólogo sueco, fue nombrado
encargado del Programa de Cáncer de la Organización Mundial de la
Salud (OMS) y fue quien puso el tema del alivio del dolor dentro de las
prioridades del programa, comenzando a principios de la década de los
80 el ambicioso plan de lograr un mundo libre de dolor por cáncer en el
año 2000. (Fig. 1)
La visión del Dr. Stenward comenzó a concretarse con la ayuda
del anestesiólogo inglés Dr. Sweerdlow, quien por su parte, logró
involucrar a destacados pioneros en el mundo del tratamiento del dolor,
tanto de USA , como de Italia y del Reino Unido a fin de concretar el
primer encuentro con relación a este tema en Milán, durante el año 1984.
En este trabajo se unieron dos tradiciones de investigación
distintas, pero que lograron complementarse muy bien. Una fue la
escuela norteamericana del Sloan Kattering Center de Nueva York; y la
otra fue la escuela europea del Saint Christopher’s Hospice de Londres,
quienes fundieron sus experiencias para llegar a un consenso en relación
a la importancia del uso de los opioides en estos pacientes. Todo esto en
un entorno de atención individualizada bajo estricto control médico.
Los Doctores John Bonica (fundador de la IASP), Vittorio
Ventafridda (fundador de la EAPC), Kathleen Foley joven neuróloga del
Sloan Kattering Center de Nueva York y Robert Twycross del Saint
Christopher’s de Londres fueron los expertos de la primera iniciativa
liderada por el Dr. Jan Stenward y el Dr. Swerdlow. (Fig. 2)
Esta primera reunión de expertos concluye con la recomendación
de un método simple, con bases cientificas, y fácilmente replicable
conocido como la “Escalera Analgésica de la OMS”. Sus principios
son el uso de opioides y otros fármacos por la boca, por reloj, para el
individuo y con atención al detalle.
Después de la reunión de Milán en 1982, se diseñaron numerosos
estudios observacionales en miles de pacientes con cáncer y dolor en
varios paises, con el objetivo de ser reevaluados 2 años después .
Grupos como el de Ventafridda en Milán y Takeda en Saitama,
lograron mostrar utilidad del método al aplicarlo en cerca de 1.400
pacientes en los cuales se encontró eficacia analgésica entre 70 y 87 %.
Las primeras recomendaciones se publicaron desde la OMS en
1986, después se superar resistencias internas en la organización sanitaria
para la recomendación entusiasta de usar opioides en la población
oncológica (11).
La difusión de la Escalera Analgésica de la OMS fue seguida de un
gran impacto en el consumo de opioides los años que siguieron en
algunos paises del mundo desarrollado, determinándose una brecha aún
mayor en el alivio del dolor entre paises ricos y pobres .
Este método relaciona la intensidad del dolor con familias de
drogas específicas para cada etapa.
Primer Peldaño =AINEs más coadyuvante para el dolor leve.
Segundo Peldaño = AINEs más opioides débiles y coadyuvantes
para el dolor moderado.
Tercer peldaño = AINEs más opioide potente y coadyuvantes para
el dolor severo. (Fig. 3)
Validación posterior de la Escalera Analgésica OMS.-
Un importante estudio de validación fue publicado por Grond en
1995.
A 10 años de la publicación de las recomendaciones se evaluó el
tratamiento de 2.118 pacientes seguidos durante 140.478 dias en los que
se utilizó el peldaño 1 OMS en el 11%; el peldaño 2 OMS en el 31%; y el
peldaño 3 OMS en el 49%. El uso de la vía oral fue 82 %, parenteral 9%,
espinal 2 % y otras vías en el 6 %. Recibieron Morfina el 56%, fármacos
coadyuvantes de la analgesia el 37% y tratamiento de otros síntomas el
79 %. Hubo uso de quimioterapia o radioterapia paliativa en el 42 %,
bloqueos nerviosos en el 8%, sicoterapia en el 3 %. La eficacia analgésica
fue buena en el 76 %, regular en el 12 %, e insatisfactoria en el 12 %.
Complicaciones como trastornos cognitivos, náuseas, constipación
y anorexia se reportaron en alrededor del 22 % para cada síntoma.
La conclusión por los autores es: marcada eficacia y bajo índice de
complicaciones utilizando la Escalera Analgésica OMS ( 7 ).
Durante el mismo año, Jadad A. publica en JAMA una revisión
sistemática de los estudios publicados en el tema eficacia de la Escalera
Analgésica entre 1982 y 1995. Se seleccionaron finalmente 8 estudios
evaluables, no lográndose realizar un meta-análisis, ya que resultaron
series sin grupo control, retrospectivas, en los cuales las condiciones
utilizadas para evaluar el dolor no fueron comparables; los seguimientos
fueron breves y las tasas de abandono altas. No obstante, se logró
determinar una analgesia adecuada global de 69 % de los pacientes
analizados. La conclusión fue que si bien los estudios eran valiosos para
dar informacion sobre el desarrollo del dolor por cáncer y su
tratamiento, las pruebas eran insuficientes para comprobar con seguridad
la eficacia de la Escalera Analgésica OMS y basarse en ella para
diseñar políticas y programas (3 ).
A fines de los 90, muchos especialistas en Cuidados Paliativos
argumentaban que las recomendaciones OMS, aunque fueron
actualizadas durante 1990, no iban a la par con los progresos en
investigación en dolor y oncología (12).
Ambitos de utilización del modelo Escalera Analgésica.-
Ademas de la adopción de este método para el manejo del dolor
oncológico en los adultos, a lo largo de estos años se ha reportado su
utilidad en otras poblaciones como son los pacientes con SIDA, cuyos
resultados son menos homogéneos por la variabilidad en el dolor y en los
tratamientos usados en dichos pacientes (10); población con cáncer
infantil con reportes de analgesia efectiva con la Escalera (6) y en dolor
de la etapa terminal de nefrópatas crónicos, publicado recientemente en la
literatura (13).
Evolución del concepto de la Escalera.-
El principal beneficio del método fue la sencillez y la posibilidad
de ser usada en medios de recursos limitados, pero que tuvieran acceso a
disponer de formas simples de codeina y morfina oral. A lo largo de
estos 20 años algunos progresos y cambios conceptuales han ido
enriqueciendo la aplicación de esta metodología:
1.- El desarrollo de formulaciones opioides orales de liberación
prolongada a fines de los 80.
2.- La entrada del tramadol como alternativa en el Segundo peldaño a
principios de los 90.
3.- El uso de la vía subcutánea para opioides en bolos o infusión
continua.
4.- El desarrollo de nuevos coadyuvantes para el dolor neuropático
( gabapentina, pregabalina ) y para el dolor óseo (bifosfonatos ) y para la
depresión (nuevos ISRS).
5.- La revolución de los COXIBs dentro de los AINEs.
6.- La validación de los procedimientos intervencionistas en algunas
situaciones.
Cuestionamientos pendientes.-
El análisis crítico del uso de la Escalera Analgésica OMS ha sido
centrado en varias interrogantes:
1. ¿Es justificable el uso del Primer peldaño ?
2. ¿Debe usarse siempre el Segundo peldaño de la Escalera.?
3. ¿Es racional el uso de opioides fuertes como primera opción en
algunos casos?
4. ¿Los métodos intervencionales deben ser utilizados siempre después
de dosis máximas de opioides potentes del Tercer peldaño ?
5. Mientras el 70 al 90 % de los pacientes obtendrían un alivio
satisfactorio del dolor según las estadísticas ¿qué pasa con el 10 a 30 %
de los pacientes que no lo hacen?
1.- Utilización de AINEs.-
La utilidad de AINEs solos o en combinación con opioides fue
estudiada en una revisión por el grupo de Mac Nicol en 3.084 pacientes.
Los AINEs resultaron mejores que placebo sin diferencia en efectos
secundarios (7 articulos). Los AINEs muestran algunas diferencias entre
sí en relación a efectos adversos (4 estudios).
La combinación de AINEs con opioides en distintas formas,
comparado con el uso de los fármacos por separado no revela ventajas o
muestra una mejoria levemente significativa en la comparación hecha en
23 estudios de corta duración (13).
2.- Justificacion del Segundo peldaño.-
Algunos autores opinan que la práctica de usar el Segundo peldaño
en forma sistemática tiene algunos inconvenientes. El paso del peldaño 1
al 2, no siempre significa mejoria en la analgesia segun Mac Nicol y otros
(14).
Muchas veces se prolonga demasiado el tiempo en que el paciente
permanece en los primeros peldaños, por temor al uso de opioides más
potentes o retardar el uso de morfina . Esta práctica se asocia a
desventajas importantes en los pacientes que necesitan titulación más
rápida de la analgesia por dolor rapidamente progresivo, o permanecen
mucho tiempo por distintas razones, con alivio insuficiente del dolor con
opioides débiles antes de cambiar de peldaño.
3.- Utilización de opioides potentes desde el inicio.-
Ha habido varios estudios recientes que evalúan el beneficio del
uso de opioides potentes desde el inicio, comparado con el uso rutinario
del peldaño 1 y 2 en 100 pacientes terminales con dolor leve a moderado.
El grupo observado, no sólo presentó mejor calidad de analgesia,
sino que necesitó menos cambios en la terapia y reportó mayor
satisfacción con el tratamiento. No hubo diferencias en performance,
status, ni en calidad de vida en ambos grupos. Para los autores, este
estudio sugiere que los opioides potentes como primera línea de
tratamiento en pacientes con dolor por cáncer terminal son seguros, bien
tolerados y útiles en esta indicación. (Marinangeli 16 ).
4.- Rol de procedimientos invasivos en dolor por cáncer.-
Tradicionalmente el uso de métodos invasivos estaba indicado
cuando el tercer peldaño de la escalera analgésica estaba agotado (una
especie de cuarto peldaño). Hoy existe suficiente evidencia que sostiene
la indicación de bloqueo neurolítico de Plexo Celiaco en dolor visceral
por cáncer localizado en hemiabdomen superior especialmente por
Cáncer de páncreas. Hay ventajas de este tratamiento sobre los métodos
convencionales de uso de opioides potentes en dosis crecientes, en cuanto
a eficacia analgésica, así como en disminución de la necesidad de
opioides, mejoría en la calidad de vida, e incluso en aumento de
sobrevida de pacientes.
Wong demostró en un grupo de 100 pacientes con cáncer de
páncreas irresecable, una mayor disminución del dolor en aquellos
intervenidos con bloqueo de plexo celiaco, en relación al grupo con
analgesia sistémica más inyección de placebo. El seguimiento fue de 1
año o hasta la muerte de los pacientes (17).
Por otra parte, Oliveira y colaboradores reportan que el bloqueo
resulta en mejor analgesia ( P 0,004 ), menor consumo de opioides
(P 0,002) y mejoría de la calidad de vida ( P 0,006) comparado con el
tratamiento standard en un grupo de pacientes con cáncer de páncreas
avanzado (18).
Hoy en día los procedimientos se definen como coadyuvantes del
manejo del dolor en cualquier etapa y no como último recurso.
Otras opciones: Algoritmo alternativo para el manejo del Dolor por
Cáncer.-
A la luz de lo expuesto, varios autores sugieren que la Escalera
Analgesica de la OMS requiere algun afinamiento para optimizar su
utilidad a la luz de la experiencia acumulada durante estos años .
Dolor leve Peldaño 1 = AINEs
Dolor moderado Peldaño 2 = bajas dosis de opioides potentes
Dolor severo Peldaño 3 = opioides potentes
Procedimientos invasivos = en cualquier etapa según corresponda
No usar opioides débiles, por su beneficio limitado, salvo que no haya
disponibilidad de opioides potentes, o el médico no esté entrenado en su
uso. Se necesitan más estudios prospectivos en mayor número de
pacientes para validar la eficacia y seguridad de este algoritmo propuesto
(1).
Conclusión.-
La utilizacion de la Escalera Analgésica OMS va de la mano con el
éxito de una de las Políticas de Salud Pública mundial más importantes
de las últimas décadas, cual es el alivio del sufrimiento de millones de
personas con cáncer avanzado.
Su racionalidad, sencillez y sus bases científicas la han convertido
en una herramienta útil para la enseñanza y la difusión de los conceptos
que permiten un equitativo acceso al alivio del dolor por cáncer en los
habitantes de todos los lugares del planeta.
La experiencia acumulada y los progresos en el conocimiento están
favoreciendo el perfeccionamiento de este método para el futuro
inmediato (19).
