Dice un conocido proverbio africano: “Si quieres ir rápido, ve solo. Si quieres
llegar lejos, ve acompañado”.
Hace unos meses comenzamos nuestra andadura juntos. Desde las
comisiones de residentes y nuevos especialistas de AEFA y AEBM-ML nos
propusieron participar en otro proyecto. Por supuesto dijimos que sí. Era la
oportunidad de participar como editores de la revista científica Laboratory Medicine
at a glance, y además estrechar cada vez más los lazos entre las dos sociedades.
Esto era lo que queríamos, colaborar para mejorar unidos.
La palabra "colaborar" viene del latín collaborare y significa "trabajar juntos en
un proyecto". La colaboración es inherente al ser humano, aunque no siempre se
consigue. Muchas actitudes pueden derribarla: la inseguridad, la vergüenza, el
desconocimiento, la falta de comunicación…
Y esto es un problema, en cualquier lugar, en cualquier profesión. En la nuestra
es particularmente un problema histórico.

paciente. Debe implicarse en el paciente. Debe ofrecer su conocimiento. Porque nuestro trabajo, nuestro
esfuerzo, aunque nos pueda parecer que no, cambia vidas.
Todo esto lo conocemos, no es algo nuevo. Cada vez los profesionales del laboratorio somos más
conscientes. Y esto lo transmitimos a los que van llegando, a aquellos que son el futuro de la profesión. La
profesión debe evolucionar. Debe hacerlo unida. Y este es el único camino.

Mujer de 72 años que ingresa en la UCI por
intoxicación por monóxido de carbono. Presenta
obnubilación y malestar general tras haber pasado
todo el día con una estufa de carbón encendida. En la
analítica realizada al ingreso, se observa elevación de
carboxihemoglobina, troponina y lactato,
sospechando una co-intoxicación por cianuro. Como
tratamiento, se le coloca una mascarilla con reservorio
de oxígeno, sueros e hidroxicobalamina.
La hidroxicobalamina se utiliza como antídoto
para tratar la intoxicación por cianuro, sustancia
altamente peligrosa para el organismo al unirse a la
enzima citocromo oxidasa. Esta enzima es
fundamental para el metabolismo energético de las
células. Cuando se une al cianuro, su función queda
anulada provocando la muerte celular. Los daños más
graves suelen ser a nivel cardíaco y del sistema
nervioso. Diversos estudios demuestran que a mayor
daño tisular, peor pronóstico con aumento
considerable de la mortalidad.
Dicha intoxicación suele producirse por la
exposición al humo como es el caso de esta paciente.
También puede darse al respirar o tragar cianuro, así
como por contacto con piel y/o mucosas.
Este antídoto reacciona con el cianuro del
organismo formando cianocobalamina, compuesto no
venenoso y soluble que se elimina fácilmente por
orina.
La hidroxicobalamina es de color rojo intenso
por lo que los pacientes a los que se les administra,
suelen presentar coloración rojiza en la piel y las
mucosas hasta un máximo de 15 días. También se
observa esta coloración en muestras de sangre y orina
del paciente pudiendo confundirse con hemólisis o
hematuria.
La coloración puede provocar interferencias en
diversas determinaciones bioquímicas tales como

Cyanide co-poisoning was suspected after finding
carboxyhemoglobin, troponin and lactate elevated in
the analysis made at admission. Treatment was based
on oxygen therapy, saline and hidroxycobalamin.
Hidroxycobalamin is used as an antidote for
cyanide poisoning. Cyanide is a highly dangerous
substance due to its binding with cytochrome oxidase
enzyme, which is essential for the cellular energy
metabolism. Once the enzyme binds to cyanide, its
function is annulled causing cell death. The most
serious damage tends to be at heart level and the
nervous system. Several studies have shown that the
greater the tissue damage, the worse the prognostic
is, with higher mortality.
Such poisoning can happen because of smoke
exposure as in the present case. It can also occur
when breathing or swallowing cyanide as well as skin
and/or mucosae contact.
This antidote reacts with the cyanide in the
organism resulting in cyanocobalamin, a non-
poisonous and soluble compound that is easily
excreted in urine.
Hidroxycobalamin has an intense red colour,
which gives patients reddish skin and mucosae up until
15 days after treatment. This colouring is also
presented in blood and urine samples that can be
mistaken as haemolysis or haematuria.
On the one hand, colouring may interfere in
biochemistry assays such as bilirubin, creatinine,
amylase, alanine y aspartate aminotransferases (ALT,
AST), iron, phosphor and creatine kinase (CK),
depending on the analyser used. These interferes can

bilirrubina, creatinina, amilasa, alanina y aspartato
aminotransferasas (ALT, AST), hierro, fósforo y
creatin kinasa (CK) dependiendo del analizador. Estas
interferencias pueden dar lugar tanto a resultados
falsamente aumentados como disminuidos, lo que
podría conducir a una interpretación errónea de estas
pruebas si se desconoce que la causa de la coloración
es debida a una interferencia causada por el
tratamiento con hidroxicobalamina y no por un
proceso de hemólisis.
Todo esto puede dar lugar a una incorrecta
valoración del estado real del paciente así como de su
evolución.
En el caso presentado se descarta la hemólisis
del suero, mediante la determinación del índice de
hemoglobina en un analizador con lectura
espectrofotométrica, tras observar un color rojizo del
suero como se puede observar en la figura. No
recibimos muestra de orina de la paciente.
La coloración en el suero desapareció
paulatinamente hasta dejar de observarse en el cuarto
día.
La evolución de la paciente fue favorable,
probablemente por no presentar daños orgánicos
graves. Recibió el alta hospitalaria al quinto día de
ingreso.
lead to falsely high or low results. This can lead to a
misinterpretation of the lab test if the cause of the
redness is not known to be a consequence of
interference due to treatment with hidroxycobalamin
instead of haemolysis. Therefore, this can lead to a
wrong interpretation of the actual status of the patient
as well as their evolution.
In the present case, we ruled out serum
haemolysis by measuring the levels of haemoglobin in
a spectrophotometric analyser, after observing a
reddish serum colour as in the figure. No urine sample
was received.
Serum colouring gradually faded out until the
fourth day. The patient’s progress was favourable,
probably because no serious organ failures were
presented. On the fifth day, the patient was
discharged.

Varón de 15 años procedente de Gambia es
remitido al Servicio de Digestivo por dolor abdominal
y diarrea sanguinolenta de cuatro años de evolución.
Se solicita estudio de heces (parásitos y coprocultivo:
negativos) y gastroscopia, donde se objetiva la
presencia de Helicobacter pylori, para el que recibe
tratamiento. Además, se realiza una colonoscopia y
biopsias, que muestran cambios compatibles con
enfermedad inflamatoria intestinal (EII) sugestivo de
colitis ulcerosa (CU).
Un año más tarde, se repite la endoscopia
digestiva y las biopsias, cuyos resultados sugieren la
presencia de enfermedad de Crohn, según el aspecto
endoscópico y la histología (ileocolitis crónica
parcheada con destrucción de las criptas, erosión y
ulceración sin disminución del componente
mucosecretor).
Durante el seguimiento se solicitó calprotectina
[458 mg/Kg; 1463 mg/Kg; VR<50 mg/Kg], apoyando el
diagnóstico de EII, y analíticas sanguíneas
destacando: fosfatasa alcalina [493 UI/L; 272 UI/L;
VR: 40-129 UI/L], gamma-glutamiltransferasa [134
UI/L; 233 UI/L; VR: 0-29 UI/L] y alanina-
aminotransferasa [47 UI/L; 63 UI/L; VR:0-41 UI/L].
Ante la alteración hepática se solicitó
enteroresonancia magnética, informándose como
posible inicio de colangitis esclerosante primaria
(CEP), y estudio de autoinmunidad: Anticuerpos
antinucleares (ANA) positivos con patrón moteado a
1/160; Anticuerpos antimúsculo liso (ASMA) positivos
a 1/80; Anticuerpos contra el citoplasma de neutrófilos
(ANCA) positivos con patrón pANCA (no se realizan
diluciones) (Figura 1), todos ellos por
inmunofluorescencia indirecta (IFI). Como resultado
de la positividad de estas pruebas se determinaron:
Anticuerpos contra antígenos extraíbles del núcleo
(ENA) por ensayo de inmunoabsorción ligado a
enzimas (ELISA) y F-actina por Inmunoblot que
A 15-year-old man from the Gambia is referred
to Digestive Service for abdominal pain and bloody
diarrhea of four years of evolution. Stool study are
requested (parasites and coproculture: negative) and
gastroscopy, where the presence of Helicobacter
pylori is found, receiving treatment. In addition, a
colonoscopy and biopsies are performed, which show
changes compatible with inflammatory bowel disease
(IBD) suggestive of ulcerative colitis (UC).
One year later, gastrointestinal endoscopy and
biopsies are repeated, the results of which suggest the
presence of Crohn’s disease (CD), depending on
endoscopic appearance and histology (chronic
ileocolitis patched with destruction of the crypts,
erosion and ulceration without reduction of the
mucosecretor component). Calprotectin was
requested during follow-up [458 mg/Kg; 1463 mg/Kg;
VR<50 mg/Kg], supporting the diagnosis of IBD, and
blood tests highlighting: alkaline phosphatase [493
IU/L; 272 IU/L; VR: 40-129 IU/L], gamma-
glutamyltransferase [134 IU/L; 233 IU/L; VR: 0-29
IU/L] and alanine aminotransferase [47 IU/L; 63 IU/L;
VR: 0-41 IU/L]. According to the hepatic alteration,
magnetic resonance enterography was requested,
reporting as a possible start of primary sclerosing
cholangitis (PSC), and autoimmunity study: positive
speckled pattern antinuclear antibodies (ANA) at
1/160; positive smooth muscle antibodies (SMA) at
1/80; positive antineutrophil cytoplasmic antibodies
(ANCA) with pANCA pattern (no dilutions were made)
(Figure 1) all of them by indirect immunofluorescence
(IIF). As a result of these tests were also determined:
autoantibodies to extractable nuclear antigens (ENAs)
by enzyme-linked immunosorbent assay (ELISA) and
F-actina by immunoblot that were negative,
myeloperoxidase (MPO) and proteinase-3 (PR3-
ANCA) were determined by ELISA being positive for
PR3-ANCA [294.9 U/mL; VR: 0-7 U/mL]. In view of the

resultaron negativos, mieloperoxidasa (MPO) y
proteínasa-3 (PR3-ANCA) por ELISA siendo positivo
para PR3-ANCA [294,9 U/mL; VR:0-7 U/mL]. Ante la
positividad para ANCA y PR3-ANCA se solicitó la
determinación de anticuerpos anti-Saccharomyces
cerevisiae (ASCA), útiles en el diagnóstico diferencial
de CU y EC, resultando negativos.
La EII es una alteración inflamatoria del tracto
gastrointestinal. Las formas más representativas son
la CU y EC. Su diferenciación es importante, pero el
diagnóstico basado en criterios endoscópicos e
histológicos no siempre es fácil1. Debido a que las
pruebas endoscópicas y biopsias son invasivas,
costosas y lentas, se han buscado marcadores
serológicos que ayuden al diagnóstico diferencial de
las EII, especialmente en casos dudosos. Los mejor
estudiados son los ANCA perinucleares atípicos
(aANCA) y los ASCA que ayudan a diferenciarlas. La
positividad de ASCA está relacionada con EC (68% de
pacientes) y aANCA con CU (40-80% de pacientes)2.
El patrón cANCA en IFI es marcador, principalmente,
de granulomatosis de Wegener y su principal antígeno
es PR3-ANCA. El patrón pANCA está asociado a
poliangeitis microscópica, glomerulonefritis
rápidamente progresiva idiopática “pauciinmune” y
síndrome de Churg-Strauss y su principal antígeno es
MPO. En las EII la especificidad de los ANCA no está
bien definida. Estudios recientes muestran una
posible asociación entre la presencia de PR3-ANCA y
EII, y su posible uso como marcador diagnóstico en
estas enfermedades, principalmente en la CU3-4. La
CEP se encuentra asociada a las EII, en el 79-80% de
los casos de CU5. Algunos estudios muestran que
PR3-ANCA también puede ser marcador de CEP en
el diagnóstico diferencial de hepatopatías6. La
positividad de PR3-ANCA varía dependiendo del
método. La tasa de casos positivos es del 29,3-31,1%
en pacientes con CU frente al 1,9-2,7% en pacientes
con EC mediante quimioluminiscencia (CIA)

The result was negative.
IBD is an inflammatory disorder of the
gastrointestinal tract. The most representative forms
are UC and CD. Its differentiation is important, but
diagnosis based on endoscopic and histological
criteria is not always easy1. Because endoscopic tests
and biopsies are invasive, expensive and slow,
serological markers that help the IBD differential
diagnosis have been searched, especially in doubtful
cases, being the best studied the atypical perinuclear
ANCA (aANCA) and the ASCA that help differentiate
them. ASCA positivity is related to CD (68% of
patients) and aANCA to UC (40-80% of patients)2. The
cANCA pattern in IIF is a marker, principally, of
Wegener´s granulomatosis and its main antigen is
PR3-ANCA. The pANCA pattern is associated with
microscopic polyangiitis, pauci-immune crescentic
necrotizing glomerulonephritis (GN) and Churg-
Strauss syndrome and its main antigen is MPO. In
IBD, ANCA specificity is not well defined. Recent
studies show a possible association between PR3-
ANCA and IBD, and its use as a diagnostic marker in
these diseases, mainly in CU3-4. PSC is associated
with IBD, in 79-80% of cases of UC5. Some studies
show that PR3-ANCA can also be a specific marker of
PSC in the differential diagnosis of hepatopathies6.
The positivity of PR3-ANCA varies depending on the
method and laboratory. The rate of positive cases is of
29.3-31.1% in patients with UC versus 1.9-2.7% in
patients with CD using chemiluminescent
immunoassays (CIA) (QUANTA-Flash® PR3, INOVA
Diagnostics)3-4. When using ELISA (QUANTA-Lite®
PR3, INOVA Diagnostics) this positivity decreases to
6% in UC and 0% in EC4. The diagnostic accuracy,
sensitivity, specificity and likelihood ratio of the

(QUANTA-Flash®PR3, INOVA Diagnostics)3-4.
Cuando se utiliza ELISA (QUANTA-Lite®PR3, INOVA
Diagnostics) esta positividad disminuye al 6% en CU
y 0% en EC4. La exactitud diagnóstica, sensibilidad,
especificidad y razón de verosimilitud de las diferentes
técnicas, para discriminar entre CU y EC, se muestran
en la tabla 1. La combinación PR3-ANCA-positivo
(técnica CIA y cut-off = 11,8 CU) e IgA ASCA-negativo
es la mejor herramienta para discriminar entre CU y
EC con alta especificidad y valor predictivo. En el caso
que nos ocupa, la realización de serología autoinmune
puede ser una ayuda en el diagnóstico diferencial. La
presencia de PR3-ANCA-positivo, ASCA-negativo y la
asociación con CEP apoyan el diagnóstico de CU.
En conclusión, aunque el diagnóstico de las EII
se realiza mediante criterios endoscópicos e
histológicos, en algunos casos la búsqueda de
biomarcadores autoinmunes pueden apoyar al
diagnóstico definitivo y así evitar la realización de
pruebas más invasivas y costosas.
different techniques to discriminate between UC and
CD are shown in table 1. The combination PR3-ANCA-
positive (CIA technique and cut-off = 11.8 CU) and IgA
ASCA-negative is the best tool to discern between UC
and CD with high specificity and predictive value. In
the case at hand, the realization of autoimmune
serology can be an aid in the differential diagnosis.
The presence of PR3-ANCA-positive, ASCA-negative
and the association with PSC support the diagnosis of
UC.
In conclusion, although the diagnosis of IBD is
made using endoscopic and histological criteria, in
some cases the search for autoimmune biomarkers
can support the definitive diagnosis and thus avoid
carrying out more invasive and expensive tests.

Los términos FIFO, LIFO y FEFO se usan
habitualmente en gestión (Tabla 1).
El laboratorio clínico, puede aplicarlos a
muestras y tiempos de respuesta: considerando
“caducidad” como “compromiso de respuesta”. Cada
laboratorio tiene modelos diferentes, pero la
combinación FIFO/rutina y LIFO/urgencia es
frecuente, aplicando entradas manuales específicas
para urgencias en los analizadores. Realmente
conviven dos laboratorios: rutina/urgencias.
En el presente artículo, introducimos un nuevo
concepto (RISO) que redefine esos enfoques desde
una perspectiva que aúna automatización, sistemas
inteligentes y orientación a resultados sobre el
paciente.
The terms FIFO, LIFO and FEFO are commonly
used in management (Table 1).
Can be applied to samples and response times,
when considering "expiration" as "commitment of
response". Every laboratory follows different
procedures. However, FIFO-routine and LIFO-urgency
approach are frequent. Specific manual entries for
emergencies in the analyzers are applied. There is a
coexistence of two laboratories: routine and urgency.
In this paper, we introduce a new concept
(RISO) that redefines these approaches from a
perspective that combines automation, intelligent
systems and orientation to results on the patient.

USO DE FIFO Y LIFO en contabilidad y gestión de stock
• FIFO (First in first out)
Lo primero que entra es lo primero que sale
Usadas en 1.- Contabilidad. Valora coste de las mercancías vendidas en relación al inventario final.
FIFO, da prioridad de venta a los primeros artículos almacenados.
LIFO, los últimos productos adquiridos tienen prioridad de venta.
2.- Gestión de stock. En perecederos (alimentación, medicamentos, reactivos, etc.), se usa FIFO o FEFO para no perder por caducidad. Cuando la caducidad no constituye un factor limitante, podemos aplicar LIFO, facilitando la gestión del almacén.

Informático del Laboratorio) y en robótica/cadenas
analíticas (LAS - Laboratory Automation System / SAL
- Sistema de Automatización del Laboratorio)
permitirían mejorar e innovar estos esquemas. La
sofisticación de los sistemas aumenta las
posibilidades de efectividad real, categorizando
solicitudes por su prioridad de salida en bloque
(cumplimiento de una serie de criterios) o
individualmente en función de necesidades concretas.
Proponemos un nuevo enfoque denominado
RISO (Random In, Smart Out), que prioriza el
procesamiento, verificación y validación según
fecha/hora de salida en que se necesita.
¿Qué ocurre con una entrada única de todas las
solicitudes (rutina, urgencias, hospitalización, hospital
de día, consultas de alta resolución, etc.)?. Si las
muestras son recibidas de manera aleatoria en el la-
boratorio, su procesamiento, en ausencia de un siste-
ma de gestión inteligente, no será eficiente ni ade-
cuado a las necesidades del paciente. Es necesario
definir un sistema que señalice al recepcionar la
muestra, su prioridad/necesidad del resultado por
parte del clínico. Y que sean procesadas por orden de
prioridad de salida, en lugar de por orden de entrada.
Planteamos un cambio de paradigma en los
flujos de trabajo. Frente al aumento de la velocidad de
procesamiento como eje, el objetivo no debería ser
estratificar muestras y/o reducir globalmente tiempos
de respuesta, sino adecuarlos a las necesidades
reales del paciente, minimizando tiempos cuando
tienen efecto sobre la atención prestada.
Implementar RISO es:
1. Definir prioridad de resultado. Consensuar
tiempos de respuesta y prioridad en función de
los compromisos adquiridos en cada solicitud.
Improvements in information technologies (EHR
– Electronic Health records and LIS – Laboratory
Information System) and robotics/analytical chains
(LAS - Laboratory Automation System) would allow
these schemes to become a game changer. The
sophistication of the systems increases the
possibilities of real effectiveness; requests are
categorized by their delivery priority as a block
(achievement of the required criteria) or individually
according to specific needs.
We suggest a new approach called RISO
(Random In, Smart Out), which prioritizes processing,
verification and validation according to the date/time of
delivery needed.
What happens when there is a single entry for
all the requests (routine, emergency, hospitalization,
day hospital, high-resolution consultations, etc.)? If the
samples are received randomly in the laboratory, their
processing will not be efficient or adequate to the
needs of the patient because there is not a smart
management system. We need to create a system that
differentiates the priority of the sample result on the
clinician side right when the samples are received.
This system needs to process the samples because of
their delivery priority, not because of their entry date.
We propose a paradigm shift in workflows.
Instead increasing the speed of processing as an axis,
the objective should not be to stratify samples and / or
reduce overall response times, but to adapt them to
the real needs of the patient, minimizing times when
they have an effect on the attention provided
Implementing RISO implies:
1. Define result priority. Consensus on response
times and priority according to the real need of
each request.
2. Laboratory information system (LIS). It must be

2. Sistema informático del laboratorio (SIL). Capaz
de definir categorías y gestionar flujos en
función de diversos atributos, adecuando cada
solicitud a las necesidades reales del paciente.
Incorporando la fecha/hora en la que se
requiere el resultado (fecha/hora de visita o
consulta) y actualizándola mediante mensajería
HL7 o web-service.
3. Sistema de automatización del laboratorio
(SAL). El sistema preanalítico y/o SAL será
capaz de reordenar muestras para procesarlas
según prioridad (por categoría o solicitud
personalizada). Los analizadores encolarán el
orden de procesamiento por prioridades.
Diseños ineficientes de laboratorio tratan de
resolverlo aumentando el número de equipos o de
personal. Pero es posible incrementar el rendimiento
tecnológico y mejorar la respuesta mediante análisis
de procesos e implementación de sistemas de gestión
efectivos. El modelo no supone requerimientos
excepcionales para el solicitante (acceso aleatorio
Random In) pero permite priorizar la emisión de
informes en función de las necesidades clínicas y
procedimientos consensuados (salida inteligente
Smart Out).
No siempre se necesita más máquina, sino más
inteligencia en el proceso.
No debemos medir tiempos de respuesta, sino
% de solicitudes/pruebas disponibles dentro del
tiempo de respuesta comprometido para cada
situación concreta.
El tiempo de respuesta “fijo” no es válido como
indicador en ninguna de sus modalidades. Sí lo es el
porcentaje de informes entregados en tiempo inferior
o igual al pactado (Slack time negativo).

adapt each request to the real needs of the
patient. The date/time in which the result is
required (date/time of visit or consultation) must
be included and updated through HL7
messaging or web-service.
3. Laboratory automation system (LAS). The pre-
analytical system and/or LAS will be able to
reorganize samples in order to process them
according to priority (by category or sample`s
request). The analyzers will queue the
processing order according to the samples
priority.
Some laboratories try to solve inefficiency by
increasing the equipment or personnel. However, it is
possible to increase technological performance and
improve response with process analysis and the
implementation of effective management systems.
The model does not impose special requirements for
the applicant (Random In) but allows prioritizing the
issuance of reports based on clinical needs and/or
consensual proceedings (Smart Out).
It is not always about the equipment, sometimes
it is just about intelligent processing.
Instead of measuring response times, it is better
to measure the percentage of available requests within
the committed response time for each one.
The fixed response time is not a valid indicator
of its modalities, but the percentage of reports
delivered in the agreed time or before (negative slack
time) is.

Varón natural de Ghana y residente en España
desde hace 10 años, que viaja por primera vez desde
entonces a su país. Realizó tratamiento profiláctico
antipalúdico 3 semanas antes y durante el viaje, pero
no al regreso. Tras presentar sintomatología
sugerente de malaria se le realizó un frotis de sangre
periférica observándose formas parasitarias morfoló-
gicamente sugerente de Plasmodium falciparum,
tanto trofozoitos como gametocitos, formas anilladas,
así como aisladas formas extraeritrocitarias de forma
redondeada y con acúmulos pigmentarios, cuya
morfología hacía sospechar presencia de P.malariae.
Se realizó una PCR para descartar parasitación mixta,
confirmándose la parasitación por P. falciparum y P.
malariae en baja parasitemia.
We present a case of a male from Ghana who
lives in Spain and comes back after visiting his country
for the first time in 10 years. He received malarial
chemoprophylactic treatment for 3 weeks (before and
during the trip), but not after the return. He presented
to the emergency department of our Hospital with
suggestive symptoms of malaria infection. Microscopic
examination of peripheral blood smear revealed many
forms morphologically suggestive of Plasmodium
falciparum, including trophozoites, gametocytes, ring
forms, as well as isolated rounded shape forms with
pigmentary accumulations suggestive of P.malariae. A
polymerase chain reaction (PCR) assay was
performed to rule out mixed infection, detecting P.
falciparum and P. malariae in low parasitemia.

La malaria o paludismo es la enfermedad
tropical importada que con más frecuencia se
diagnostica en España1, presenta una mortalidad del
2-3% en gran medida por retrasos en el diagnóstico e
inicio del tratamiento2. Existen 6 especies capaces de
causar la enfermedad en el hombre: P. falciparum, P.
vivax, P. ovale (P. ovale wallikeri y P. ovale curtisi), P.
malariae y P. knowlesi, aunque existen algunos casos
descritos de infecciones por otras especies no
humanas. La mayoría de los casos están causados
por P. falciparum, seguidos de lejos por P. vivax y P.
knowlesi siendo la infección mixta muy infrecuente.
El diagnóstico consta de:
A. Examen microscópico: considerado el “gold
standard”, deben de realizarse al menos 3
exámenes durante varios días antes de poder
descartar el diagnóstico de malaria, sobretodo
en bajas parasitaciones1.
Gota gruesa: Al analizar mayor cantidad de
sangre permite cuantificar el grado de
parasitación por lo que es útil para valorar la
respuesta al tratamiento incluso con bajas
parasitaciones. Es la técnica microscópica
más sensible, aunque no es posible realizarla
en todos los laboratorios ya que requiere de
personal entrenado y no siempre permite la
identificación de la especie.
Frotis o extensión en capa fina: Menos
sensible que la gota gruesa, también permite
obtener el grado de parasitación. Las
principales ventajas frente a la gota gruesa
son que es una técnica más sencilla de
realizar, y que al no romper los eritrocitos
permite identificar la especie infectante y las
parasitaciones mixtas (más específica).
B. Test de diagnóstico rápido: son técnicas
inmunocromatográficas para la detección de
antígenos parasitarios. No permiten la

disease diagnosed in Spain1, with a mortality of 2-3%
frequently related to delays in diagnosis and
treatment2. There are 6 known species that can affect
humans: P. falciparum, P. vivax, P. ovale (P. ovale
wallikeri and P. ovale curtisi), P. malariae and P.
knowlesi. There are also a few cases reporting
infections from other nonhuman species. The majority
of cases of imported severe malaria are caused by P.
falciparum, followed distantly by P. vivax and P.
knowlesi. The mixed infection is very infrequent.
The diagnosis of malaria consists of:
A. Microscopic examination: considered the “gold
standard”. At least 3 microscopic examinations
in several days should be performed until
malaria is definitively excluded from the
differential diagnosis, especially with low levels
of parasitemia1.
Thick blood smear. The examination of a
large sample of blood allows parasite
quantification and monitoring treatment even
with low levels of parasitemia but does not
always allow the identification of the species.
It is a complex technique that requires highly
trained personnel.
Peripheral thin blood smear. With lower
sensitivity but higher specificity. As thick
smear, it allows parasite quantification and
monitoring treatment. Technique is simpler,
and because it does not break the
erythrocytes allows the identification of the
species and mixed infections.
B. Rapid diagnostic test: there are several
immunochromatographic techniques for the
detection of parasite antigens. The principal
disadvantages of these methods are that they
do not allow parasite quantification, not being

cuantificación de la parasitemia por lo que no
son útiles para evaluar la respuesta al
tratamiento y pueden tener falsos negativos en
bajas parasitemias o en muy altas por efecto
prozona3.
C. Pruebas de biología molecular: los más
comunes son técnicas basadas en la PCR
(convencional o en tiempo real), muy sensibles
y útiles como pruebas confirmatorias de
especie, parasitemias mixtas y
submicroscópicas4. Tienen un elevado coste,
lentas (entre 6 y 24 h) y no disponibles en todos
los laboratorios5.
D. Técnicas serológicas: no son útiles en casos
agudos de malaria.
Debido a la gravedad de la enfermedad, el
diagnóstico de malaria es siempre urgente, siendo
recomendable disponer del resultado en menos de 3h
para no retrasar el inicio del tratamiento. La clínica de
la malaria importada no siempre es específica, los
síntomas más comunes son fiebre, cefalea y
artromialgias, aunque puede haber pacientes que no
presenten fiebre, por lo que ante la sospecha se debe
extraer una muestra de sangre inmediatamente, haya
o no fiebre. El estudio microscópico sigue siendo el
“gold standard” para el diagnóstico aunque se puede
utilizar un test de diagnóstico rápido como prueba
inicial de cribado (nunca sustituir) cuando no esté
disponible.
useful for evaluation of response to treatment
and the possibility of having false negatives in
low parasitemias or in very high ones due to
prozone effect3.
C. Nucleic acid detection: most common methods
for detection of Plasmodium spp. nucleic acid
are PCR-based techniques (conventional and
real-time), very sensitive and useful as
confirmatory tests for identification of species,
mixed and submicroscopic infections4. These
are high cost techniques, with long response
time (6 to 24h) and are not available in all
laboratories5.
D. Serological techniques: they have no utility in
diagnosing acute cases of malaria.
The diagnosis of imported malaria is always
urgent, it is recommended to have the result in less
than 3 hours in order to avoid a delay in treatment.
Malaria symptoms are not always specific, most
common are fever, headaches and muscle pains, but
some patients do not always present with these.
Laboratory testing should be requested as soon as
malaria is suspected, whether or not there is a fever.
Microscopic examination remains the “gold standard”
for the diagnosis, when is not available, a rapid
diagnostic test can be used as an initial screening test
(never replace).
