¿Porqué fracasan los Ac monoclonales?
• Vías redundantes de supervivencia tumoral
• Ambiente inmunosupresor tumoral y elementos reguladores.
• Interacción subóptima con NK, T-CD4+, y T- CD8+
• Competición con IgG circulantes

• Pros:
– Enorme eficacia
– Tiempo de respuesta 1 mes
– RFS: 8 meses
– No síndrome de liberación de citocinas
• Contras
– EAs neurológicos: temblor , confusión, déficits motores focales, aunque reversibles.
– Muy caro y de dificil logistica

Hampers proliferation and function of CD8+ T-lymphocytes through inhibition of T-cell receptor signaling
Blockade of early signal transduction events in T-cells Inhibits the activation and
proliferation of normal T-lymphocytes in vitro
Suppression of T-cell function (Lck-mediated)
Impairs the induction of specific cytotoxic T-cells

Suppression of NK cytotoxicity Induces CD34+ cells to produce DCs
Impaired capacity of dendritic cells to elicit primary T-cell responses
Having a large granular lymphocytosis while on dasatinib has been associated with better response
Ameliorates GVHD in CML BMT patients
Low incidence of infections

– Tec (Bmx, Btk, Itk, Tec, Txk)
– Syk (Syk and Zap-70)

• Physiological function
– PD1 expression is induced when T cells become activated
– PDL1 induced with IFN i and II and TNFα
– When engaged with ligand:
• PD1 inhibits kinases that are involved in T cell activation
– It limits the activity of T cells when infection occurs and limits
autoimmunity
• inhibits the TCR ‘stop signal’, modifying the duration of:
– T cell-APC or T cell–target cell contact
• Augments Tregs proliferation
• TH1 cells produce IFN gamma

Retreatment With Nivolumab
Pretreatment 6 weeks posttreatment Progression when therapy stopped

Conclusions
• With longer follow-up, treatment with nivolumab in patients
with cHL indicated:
– Consistent and manageable adverse event profile
– Both CRs and PRs were durable
– In the one patient that was retreated post-progression, a second
response was achieved
• Long-term therapy (up to 2 yr) appears feasible and is
associated with durable responses, and encouraging PFS and OS
• These data supported CHECKMATE 205 a larger, ongoing phase
2 study.
