RECOMENDACIÓN
El fármaco recomendado para el tratamiento a largo plazode la trombosis en pacientes con cáncer es HBPM durante
al menos 6 meses (1A)
Considerar prolongar el tratamiento más de 6 meses en pacientes de alto riesgo como aquellos con cáncer activo
y/o tratamiento quiomioterápico (2C)

RECOMENDACIÓN
La EP incidental debe tratarse con tratamiento anticoagulante como la trombosis sintomática (1C)
En la trombosis incidental del territorio esplácnico sintomática se realizará tratamiento anticoagulante. Si
es asintomática se individualizará en cada paciente (2C)

ü No retirar si funciona, está biencolocado, no está infectado y latrombosis se resuelva adecuadamente
ü Si es necesario retirarlo, no antes de 5-7días de tratamiento antitrombótico
NUEVOS ANTICOAGULANTES
No existen datos suficientes para indicar los nuevos anticoagulantes orales en pacientes con cáncer
CONCLUSIONESü El fármaco de elección: HBPM
ü La duración del tratamiento no está bien establecida. Se aconseja un mínimo de 6 meses
ü La EP asintomática debe ser tratada como la trombosis sintomática. La trombosisincidental en territorio esplácnico recibirá tratamiento anticoagulante si es sintomática. Enlos demás casos individualizar la actitud para cada paciente.
ü Se aconseja anticoagulación a dosis plenas si plaquetas ≥50x109/L, profiláctica si 20-50x109/L, y evitar la anticoagulación si <20x109/L. Durante el primer mes transfundirplaquetas para conseguir recuentos ≥50x109/L y mantener dosis plenas deanticoagulación
ü Si CrCl < 30 mL/min, monitorizar la función renal y la actividad antiXa. La HNF o los AVKpueden ser una alternativa en esta población si no se dispone de antiXa
ü La trombosis de catéter venoso central debe seguir el tratamiento entándar. No retirar si esnormofuncionante, no está infectado y hay respuesta al tratamiento anticoagulante
ü Los pacientes con tumores cerebrales reicibirán tratamiento antitrombótico estándar.
