Con mas de 70 años de existencia, el Servicio Nacional de Salud del Reino Unido (NHS) ha logrado una posición icónica para los ciudadanos y es reconocido por los académicos que comparan los sistemas de salud a nivel global. Desde lejos puede parecer que el servicio ha permanecido esencialmente sin cambios desde su nacimiento en 1948, pero esa idea esta lejos de la verdad. Lo que ha permanecido es la esencia del Sistema, que está financiado por impuestos nacionales y ofrece cobertura universal, que en la mayoria de los casos se brinda en el lugar de la atención sanitaria. Pero Gobiernos sucesivos han realizado cambios en la estructura, en los modelos de incentivos y en la regulación del Sistema, buscando mejores resultados.Ese proceso continúa dentro del NHS que enfrenta varios desafíos en 2019 y hacia el futuro. Estos desafíos pueden ser familiares para ustedes en sus propios países o servicios. El foco de este programa es brindar una oportunidad para discutir esos desafíos, escuchar a personas que trabajan en diferentes niveles del NHS contando como enfrentan esos problemas, y compartir expectativas para el futuro según el rol que cada uno desempeña y, siempre que sea posible, hacer un aporte positivo. Ejemplos de los desafíos que vamos a discutir incluyen,

• ¿Cómo gestionamos el aumento de los costos manteniendo, y mejorando en lo posible, la calidad de los servicios y la seguridad del paciente?

• ¿Cómo balanceamos nuestros esfuerzos entre la prevención y el tratamiento?

• ¿Cómo construímos el Sistema sobre la excelencia de nuestra Atención Primaria, si estos servicios, como muchos otros, enfrentan una crisis de recursos humanos? ¿Qué estrategias podemos usar para desarrollar el talento humano que exigirá el futuro de los servicios?

• ¿Qué rol debería tener el Sector Privado en nuestro Servicio Nacional de Salud? ¿Cómo mejoramos los servicios para los Adultos Mayores y Salud Mental?

PRESENTACION DEL PROGRAMA

¿Cuáles son las oportunidades que surgen de la innovación y cómo podemos aprovecharlas? ¿Cómo puede transformar los servicios la Salud Digital y cuáles son los obstáculos que necesitamos superar para que esto suceda?

Nuestro Programa ha sido diseñado para considerar todas estas cuestiones. Usted podrá conocer e interactuar con docentes que están dispuestos a compartir sus visiones y sus experiencias. Todos han trabajado en servicios opertativos, pero también han realizado una contribución a nivel nacional, por lo tanto, las sesiones se nutren de una profunda experiencia sanitaria. Además, hay una gran oportunidad de compartir sus conocimientos y experiencias con los compañeros del curso. Hay mucho tiempo para hacerlo durante las sesiones formales. Pero un rasgo distintivo de este Programa es la “Mesa Redonda” donde los participantes que quieren mostrar sus experiencias pueden hacerlo, culminando en una sesión donde los mejores trabajos serán presentados y premiados. Adoptamos la filosofía de “todos enseñamos, y todos aprendemos”. Muchos de los profesores, que les hablarán sobre los servicios de salud en el Reino Unido, vuelven año tras año para formar parte de este evento, porque encuentran muy estimulante el compromiso con los colegas de América Latina y un ambiente de aprendizaje continuo.

Dr. Bernard Crump Professor of Practice in Healthcare and Leadership, University of Warwick El Prof Bernard Crump estudió Medicina en la Universidad de Birmingham, obteniendo la graduación en 1980. Después de siete años de práctica clínica e investigación, retornó a los West Midlands para realizar un posgrado en Salud Pública. Durante una década fue Director de Salud Pública en South Birmingham y en Leicestershire, donde también se desempeñó como Deputy Chief Executive. En 2002 fue nombrado CEO de la Autoridad Sanitaria Estratégica de Shropshire y Staffordshire, un puesto que ocupó hasta ser designado como el primer CEO del Instituto para la Innovación y la Mejora Continua del NHS; en Julio del 2005. Este Instituto ha desarrollado una amplia gama de programas que sustentan la mejora de calidad para los servicios del NHS, como Productive Ward y Productive Series, No Delays Achiever y comisionados. Ha publicado trabajos y brindado conferencias sobre una amplia gama de temas vinculados a la Atención de Salud, incluyendo aspectos sobre salud pública, el rol de los clínicos en la gestion administrativa , la mejora de calidad, el uso de medidas estadisticas para alentar la mejora continua y la aplicación de Economía de la Salud para la toma de decisiones. Over the seventy years of its existence, the UK National Health service hasachieved iconic status from UK nationals and recognition from academicswho compare health care systems globally. From afar it may appear that theservice has remained essentially unchanged since its birth in 1948, but thatwould be far from the truth. What has endured is the essence of a systemwhich is funded from central taxation, offers universal coverage and, in mostcases, is free at the point of use. But successive Governments haveexperimented with the structure and with the incentives and regulationwithin the system as they seek improved performance. That processcontinues as the NHS faces major challenges in 2019 and beyond. Thesechallenges may be familiar to you from your own country or service.

At the heart of this programme is an opportunity to discuss these challenges,to hear from people working across the NHS about how the challenges arebeing confronted, and to share perspectives for the future as we each playour role, wherever we may make our contribution.

Examples of the challenges we will discuss include,• How to manage the rising costs of healthcare whilst at least

maintaining, and hopefully improving the quality and safety ofservices?

• How do we balance our efforts between prevention and cure?• How do we build on the excellence of our Primary Care services as

they, like many of our services, face a workforce crisis?• What role should the private sector play in our health services?• How do we insure improved services for Older people and for Mental

Health service users?

PROGRAMME OVERVIEW

• What are the opportunities arising from innovation, and how canthese be exploited?

• How can Digital Health transform services and what are the obstaclesthat need to be overcome for this to happen?

Our programme has been designed to consider all of these questions. Youwill meet and engage with a faculty that can share their views and theirexperiences. They have worked at the frontline of services, but all of themhave also made a national contribution and the sessions they will lead benefitfrom this broad experience.

In addition, there is the enormous opportunity to tap into the knowledge andexperience of fellow course participants. There will be ample time to do thisduring the formal sessions. But a feature of this programme is the“Roundtable”. Here those participants who wish to showcase their ownexperiences are able to do so, culminating in a session where selectedparticipants present on their work to the group as a whole.

We adopt a philosophy of “all teach, and all learn”. Many of those who willspeak to you about services in the UK return year after year to take part inthis event because they find the engagement with colleagues from LatinAmerica so stimulating and the atmosphere is so conducive to learning.

Dr. Bernard CrumpProfessor of Practice in Healthcare and Leadership, University of Warwick

Prof Bernard Crump studied medicine at the University of Birmingham, qualifying in 1980. After seven years in clinical practice and clinical research, he returned to the West Midlands to postgraduate training in public health medicine.

He spent a decade as Director of Public Health in South Birmingham, and in Leicestershire, where he was also Deputy Chief Executive. In 2002 he became CEO of Shropshire and Staffordshire Strategic Health Authority, a post which he occupied until being appointed the first CEO of the NHS Institute for Innovation and Improvement in July 2005. The NHS Institute has developed a wide range of programmes that are supporting the improvement of the NHS, the Productive Ward and the Productive Series, the No Delays Achiever and commissioners.

He has written and lectured on a wide range of topics in healthcare, including aspects of population health, the role of clinicians in management, health and healthcare improvement, the use of metrics in encouraging improvement and the use of health economics in decision-making. He is currently a Professorial Teaching Fellow in Medical Leadership at Warwick Medical School.

Dr Andrew Vallance-Owen

Dr Andrew Vallance-Owen graduated from the Birmingham Medical School in 1976 and was Chair of the Guild Trustee Board 2011-2013. A trained surgeon, he then joined the staff of the British Medical Association -he was Scottish Secretary and then became Head of Policy and a spokesman for both the BMA and the medical profession.

Dr Vallance-Owen has recently retired from BUPA, where he was accountable for the safety and quality of care of BUPA’s 11 million customers. At BUPA he maintained an external profile; he is a keen advocate of improved doctor-patient communications, measurement of clinical performance and shared decision making.

Dr Vallance-Owen now has a number of non-executive roles in education and in both NHS and private healthcare. He has also been promoting UK healthcare plc in developing markets overseas through his involvement in Healthcare UK based at UK Trade and Industry in Whitehall.

Dr Alvaro Barrera Parraguez, FRCPsych, MSc, PhD

Dr Alvaro Barrera Parraguez is a consultant psychiatrist at Oxford Health NHS Foundation Trust, honorary senior clinical lecturer at Oxford University, and a Fellow of the Royal College of Psychiatrists.

He completed his PhD at Cambridge and joined the neuropsychiatry rotation at Addenbrooke’s Hospital.

His research focuses on transforming inpatient mental health care using novel digital and organizational approaches. He is working on introducing optical sensors in acute psychiatric wards to improve care, as well as with the fellows from the Oxford Department of Biomedical Engineering, on digital solutions for clinical needs.

He was the PI of the first pilot RCT of CBT for insomnia on an acute inpatient unit and is editing a textbook on inpatient mental health care with Oxford University Press .

Professor Ian Philp, MB ChB, MD, FRCP, FFPH (HON)

Dr Ian Philp is Professor of Global Ageing Studies at the University of Stirling. Medical Director NHS in Warwick, Hull and Birmingham. He was professor of Health Care of Older People and Consultant in Geriatric Medicine at the University of Sheffield, and National Director of Older People in England between 2000-2008.

Presenter and developer of programmes on ageing: 100yrs Younger in 21 Days (ITV), Only the Lonely (ITV),-How to Live Longer (BBC), Are You Younger Than You Think (BBC)

Founder of the EasyCare Academy, a global social enterprise for digital learning, assessment and personalised care of older people.

Dr Philp has been granted numerous awards: CBE; Queen's Birthday Honours, 2010; Team leader, Queen's Award for Higher Education, 2000; Team leader, Hospital Team of the Year, Older People, 1998.

Dr Veronica Wilkie

Dr Veronica Wilkie is a General Practitioner (GP) in a training practice in Droitwich, Worcestershire, and a Senior Clinical Teaching Fellow at the Institute of Clinical Leadership at Warwick University Medical School.

She has a master’s degree in Evidence-based Medicine and was a GP trainer, course organiser and postgraduate tutor, before starting her current post at Warwick University.

She is currently working with the NHS Institute on evaluating the implementation of the Medical Leadership Competency Framework, and she teaches and facilitates courses on medical leadership and evidence-based practice (EBP). She is the Curriculum Guardian for the EBP statement for the Royal College of General Practitioners (RCGP) curriculum. (March 2008).

Dr Tom Hughes, Emergency Physician, Oxford

Dr Hughes is an Emergency Care Physician who trained in Australia and the UK. He practises at the Oxford University Hospitals NHS Trust.

As well as delivering Emergency care, Dr Hughes has a major interest in healthcare informatics. He holds a national role as the Clinical Lead for the Emergency Care Dataset. This project involves the agreement of a common suite of clinical data that will be collected in all of the 220 Emergency Departments in hospitals across England.

He has also developed a number of Healthcare related Apps.

Dr Charlotte Refsum

Dr Refsum was part of the National Medical Director’s Clinical Fellowship Scheme, a national scheme in which talented doctors in their early career are seconded to work directly with the leaders of national organisations, including private sector organisations.

During this time she worked with Mark Britnell, Global Chairman and Senior Partner for Health for KPMG. She was a key part of the team working with Mark in the production of his book “Human: Solving the Global Workforce Crisis in Healthcare”, which was published in Spring 2019, and which will be the subject of her session.
